Helping Families Who Are Homeless Achieve Self-sufficiency

The Pathfinder Community Connections Transitional Housing service helps families who are homeless work toward self-sufficiency and permanent housing. The service provides family programming and reduced-rent housing for up to two years, depending on the funding. The service is available in Huntington, Plymouth and Knox, Indiana. Apply now 


Eligibility guidelines include families with children who have no dwelling or resources to obtain a dwelling. Families need not be living “on the street.” Referral/Intake Process When you contact us, to help the Transitional Housing Support Specialist determine eligibility, please give the name, address and phone number at which the head of the household may be contacted, plus information about the current living situation. When possible, have the head of the household call. The potential participant will then complete an application form and Pathfinder Community Connections will check references and coordinate with other appropriate human service agencies. Children who are less than seven years old must have lead level in their blood tested. Services Provided Transitional Housing emphasizes family programming. Participants receive support from a Transitional Housing Support Specialist which includes:

  • Help to identify & contact other needed services
  • Help define options
  • Help to obtain a GED or other education
  • Help finding and keeping a job
  • Help to obtain child support
  • Help to obtain child care
  • Help with money management skills

In order to continue receiving Pathfinder Community Connections Transitional Housing, participants must:

  • Continue working on personal goals
  • Keep the home clean
  • Pay rent equal to 30% of income
